Criminal Records Check

Please download and print forms. Follow instructions on form. Disregard reference to DRO or to Guardian ad Litem - Send the results to me.

Each person 14 years of age or older who will regularly or frequently be staying or working at the home while children are being provided care, must obtain a criminal history and central registry background check. The results of those checks must be documented.

CPS Check:

Please download and notarize form - a separate form is needed for each person. After notarizing, please send to the Austin, Texas address at top of form. CPS will do the check and send me the results to me by email.
